My son is leaving on vacation with friends.  I need to write some sort of medical release so that in case anything happens they can (his friends parents) authorize medical treatment.  Any ideas on what I should write?

How about:


I (insert name) authorize (insert friend's parents name) to make decisions regarding medical treatment to my son (insert his name) when need arises.


Then sign it!

In the event that my son, (insert name), needs immediate medical attention and I am unreachable, I authorize X to make decisions on my behalf.
