The only time I had to do one I googled for an example and then just cut and pasted it into word and customized it. It just needs to be short and sweet and thanking them for the opportunity to work for them for all these years or months or whatever and how you learned so much.

ooooh, quitting annoying jobs is about as much fun as you can have at work. here's what i said when i resigned from the job i hated:
Dear (name of my boss):
This letter serves as notice of my resignation from [name of company]. My last day of work will be Thursday, May 12, 2005.
I feel fortunate to have been associated with [company name] for the last three years. My experience and training have been invaluable, and I appreciate having had the opportunity to work with so many talented people.

Cheesy? Maybe. But polite, yes. No need to burn bridges, even if you think you won't need them, you never know. Oh, and I cc'ed it to the two personnel people so they could get started on their paperwork.
