I need help for a work project. I am on a committe at my company and we are trying to implement a new employee incentive program. If any of you have any ideas or things that you may enjoy at your own job I would love to hear them. We need ideas on types of incentives other companies provide, how often incentives are given, etc.

Are you talking gifts? What would the incentives be rewarding? I know that gift cards are always appreciated. I have a friend who works for a major advertising firm and she's rewarded with spa days.
Depending on where you live, you could also do gift certificates for a local ice skating rink or a ski resort. You can always tie them into the season. For summer, maybe a gift certificate for a local water park.
The types of incentives are necessarily the hard part...it's the when/why of incentives. What does your company do? Knowing that could help us figure out some of your other questions. For instance, if it's basic retail: every sale over $100 or $200 gets X amount of gift certificate. For clinical research: the person who recruits (and enrolls) the most patients in a month gets X. Those are just the two jobs I've had in my life so that's what I based it upon.

We have "appreciation awards"= $50 and "excellence" awards = $100
HR sets a limit of 3 appreciation awards per month and 3 excellence per quarter.
When Managers see someone doing something above and beyond, they submit a little form saying why the person deserves the award. HR approves and then the employee gets a copy of the form with nice words and goes to accounting to pick up their award:
For the awards: we do gift cards (we buy a selection of gas cards, local stores and restarants and let the winning employee choose.) We even sent out an email having employees request the kinds of cards they would like to see as gifts.
Everybody feels involved in the process and the employees really like it.
Once per quarter, Mgmt gets together and votes on the employee that stood out the most - that person gets employee of the quarter and a check for $1500.
Gingembre1-The actual ideas on what the gifts would be rewarding is one of the things that is still up in the air. I love the spa idea!
relrel-We live in Texas so the water park thing is a great idea. We work at a medical device manufacturing plant, so there are many types of jobs from manufacturing to clinical, marketing to the financial departments, etc. I think that is one of the hard parts, because since everyone's jobs are so different it is hard to compare each individual based on performance. The only department that has incentives right now are the sales team, because they are counted on their own.
Basically, we used to have a president's club thing where they would pick people to send them on really nice trips every year, but then the people who didn't get to go would be very annoyed. I think they are trying to revamp it into something where more people can be awarded more often than the big expensive trips once a year.
