perioral dermatitis

Around 5 years ago, while in Germany, I woke up with these tiny zits on my chin that I couldn't get rid of - kind of like a rash. I thought that maybe something in the unfamiliar environment might have triggered it. Not knowing what it was, and thinking it was eczema, I used a low-dose OTC cortisone cream each morning and each night to keep it under control. Like I said, I've been battling this rash for like 5 years...

I'm not big on going to the doctor, and put it off.

Last Friday, I went to a dermatologist, had three tiny bumps from the rash and she immediately spotted them and told me it looks like "perioral dermatitis." So she gives me these antibiotics, and acne cream that cost me $75. I have been taking the antibiotics and using the cream since she gave it to me. From the reading I did, I didn't use any of my regular moisturizers or the cortisone all weekend in hopes of getting rid of this. Bottom line - I look like crap. The rash is horrible (worse than I ever had it, and with my regimen, you would have never been able to see it) and now it's red, bumpy, scaly, and all around my nose and chin area. I couldn't NOT wear make up over it to work today and even put cortisone cream on top of my makeup before I walked out the door because it's so itchy and looks so red and awful.

I'm super sad and embarrassed to look at anyone. I am going to continue with the pills and cream, but I can't NOT use OTC cortisone and continue to go out in public.

Since this is apparently so common, have any of you had this, and if so, could you please share your experience with it?

I am also seeing another dermatologist next week - this acne cream thing just doesn't seem logical to me.

Hmmm. I think I had this before. I had small red bumps around my mouth and a little around my nose. I didnt think it was acne because I had never had a breakout before. I went to the doctor and she told me it was a rash, most likely from my toothpaste (and after reading the description briefly, I think it was cinnamon toothpaste that I was using). She gave me cream for my face, but no pills. I started using the original flavor crest tootpaste also. It cleared up in about a week, but it never got worse, as you seem to be experiencing. I dont remember the name of the cream I was given. Actually, it was more of a gel. I wonder if she gave you too strong a dose of the medication. If youre using flavored toothpaste, mouthwash, mints, etc. I would stop using them. Just in case its keeping the rash there.

I totally understand how youre feeling though. When I had the rash I was embarassed to go out. It will clear up though. Hopefully I was a little helpful. Feel better!! biggrin

it's definitely not acne, that's why I'm confused by the acne cream. Nothing I have read has mentioned the use of acne cream (but has mentioned antibiotics.) what you had is exactly what I have - I wish you could remember the name of the gell too!

I would go to a different doctor and tell them that the medication you received is only making the rash worse. I dont know why she gave you acne cream. Since she was able to point it out so quickly, you would think she would know what type of meds to prescribe. I'll see if I still have the gel...but I'm pretty sure I tossed it.

to make a long story short, she's a D.O. and her father is an M.D. Her father's name is on the prescription and it was pre-printed. Also, she is fairly ditzy (I went to her for some cosmetic tweaks, which she is good at, but I don't trust her as a doctor to diagnose and prescribe... good thing I already had another dermatologist appointment through my primary care physician)

i went in there with three tiny bumps near my nose, and now I have a horrible rash on the lower half of my face cry

I did some quick reading around the 'net about what I found out is so affectionately termed "muzzle rash". Doesn't that make you feel good? hmm.gif Anyway, what was in the acne cream that she gave you? Based on this: http://www.aad.org/public/Publications/pamphlets/PerioralDermatitis.htm

it appears that maybe the cortisone is maybe making it worse. Perhaps the acne cream had some form of a corticosteroid in it as well and in combo with what you are using is causing the flareup. I would talk to your derm (not the ditzy one!) about perhaps taking a really low-dose antibiotic every day (even though I disagree hugely with general overuse of antibiotics) or perhaps a lowd-dose topical antibiotic. Perhaps the acne cream had salicylic acid in it which could probably aggravate the dermatitis.

Good luck! I have excema around my eyes and during the colder months it can make my eyes puffy and wrinkly...I know what it's like to not want to look anybody in the face.

i know, i saw that term - makes me feel even more ugly :(

I will continue with the antibiotics (she gave me a sample bottle and no script - another odd thing) but I'm going to do my regular regimen until I can get the right cream. Also, the cortisone that causes a problem (at least more of a problem) is the prescription strength - I'm doing low strength 1% cortisone. *sigh*

I also have a problem with it around my eyes in the winter too. when do I get a break? I mean, seriously? I guess I could have it worse - at least I can walk and I have my hands.

Sorry, no suggestions, just wanted to offer my sympathy. I'd tell you chin up, but I think in this case it's chin down (sorry, couldn't resist) wink.gif

I'm sorry. I also have a form of dermatitis. I have seborrheic dermatitis and it is so annoying!! My symptoms are similar to yours except I don't get little pimples. I just have the flaky, scaly, itching, burning flare-ups that can show up anywhere from the corners of my nose, chin, corners of my mouth, eyelids, eyebrows, hairline, around my ears, and scalp. If really bad, sometimes all of the above. Its so much FUN! blankstare And I will have it for all of my life. HOORAY!! blankstare

The doctor gave me a cream that keeps it under control (I forget the name) and I use the Nizoral shampoo, which my doctor says you can use on any flareups that show up on the face. Maybe give that a try. You can get it at any drugstore.

aw honey, I'm so sorry. that totally sucks. i'm feeling as though this will be a lifetime battle for me too. i hate that its around my eyes as well (currently have the welts around my eyes going down) I will look into the nizoral shampoo after I see the other dermatologist. i'm sticking with my OTC 1% hydrocortisone morning and night until I get better direction. it's all so frustrating and really sucks.

That sucks!  I don't think this is the same thing but may be worth a try.  I was getting a rash like that around my mouth and I switched to a flouride-free toothpaste and it made a big difference.  You didn't happen to switch to a new toothpaste did you?  Maybe you're allergic to it?
