Do you address the card to just the mommy-to-be or both the mommy and daddy-to-be?

If you're a guest?  I'd do both, because the baby is going to be both of theirs and therefore the gift is for both of them.

I usually address the card to "baby last name" - that's probably not the "etiquette" correct way, but that's what I do anyway...
