Since this is a grandmother's delicate skin that just got fried by radiation, I would go for the softest imaginable hat you could find. Honestly, I would be going the scarf route - seams and coarser fabric just sound painful.  I have bought hats for my mother (loses her hair when she goes into chemo) at TJ Maxx with pretty good success.  They also have a lot of scarves there.  Since the hat wearing is temporary, I wouldn't spend a ton of money, unless you know she'll get a lot of wear from it later.

you can also buy a standard large square scarf and tie it:
