LAST UPDATE
I start with the last update since the story has been long forgotten, just wanted to vent yet again:
I never recieved my prize or any confirmation that my notarized affidavit was received, after I re-sent it. Hard to believe that the mail would get lost TWICE....Again, contacted the editor about my disappointment - NOTHING....


Just wanted to vent:
I've been a very devoted reader & a subscriber of LUCKY magazine for years. Each month they have giveaways, i even won once 2years ago: mineral bronze make up ( bronze/shadows/blush & bronze eye liner). Was TERRIBLY excited! Screamed like a school girl!
Last March they had a month of giveaways, each day a different item to sign in for, i did & was surprised in a month that I was  picked a lucky winner of a hot iron styler for hair, retail value $119! With much excitement, I filled out the attached form, signed, notarized as requested ( to verify that I'm who I am & to cover issues for potential law suit ) & mailed back ASAP.....it's been a month, I never received a prize! I replied to the e-mail that notified me of my prize - no response....I even received a notice from editors to e-mail them pictures of winners - they'll publish them monthly....I replied, that i never received the prize....No response....I contacted the editor on LUCKY website - NO RESPONSE!!!! I'm confused & disappointed with lack of any acknowledgment of my inquiries....It's not about the prize anymore, just to receive 2 lines in e-mail why I never received the prize or that they received  my inquiry & are looking into it....


UPDATE

JUst ( it's been over a month) received a phone from the mag - they couldn't find my notarized declaration, but finally saw my inquiries & will send me the prize with the terms to re-do the form/declaration. So, i'm nor cancelling the subscription anymore:)
